In a Qualifio campaign, the form is the step used to collect participants information. It allows you to gather identification data (such as name or email) required to validate participation, contact winners or enrich your database.
A form can be added to a campaign or even embedded on an external website, and each field you include corresponds to a piece of data collected from the participant.
Creating a form
To create a form, you access the form editor, which can be found on the tab "Content" on the left side of the screen.
Then, you define its name and language. The language determines the automatic labels displayed for identification fields and ensures the form matches the campaign language.
You then add fields, which are the building blocks of the form. Different field types are available, including identification fields, single-choice, multiple-choice, open text or file upload.
Each field can be configured individually:
- make it mandatory or optional,
- define validation or format,
- add an error message,
- connect it to variables for integrations or databases.
You can also reorganise fields, hide them from users or archive them without affecting existing campaigns.
Additionally, a custom text may be displayed above or below the form to explain why information is required.

Form position in the participant journey
By default, the form appears after the questions or game and before the final screen. However, you can place it before the campaign content.
Placing it before ensures you collect data from all participants. For example, in elimination-type quizzes, users might leave after losing and never complete the form if it appears later.

Pre-filling form fields
When participant data is already known, the form can be pre-filled. In that case, users may not need to complete it manually, which reduces effort and improves data quality. This can be done through:
- URL parameters,
- JavaScript integration,
- SSO login data,
- secure tokens.
Pre-filling speeds up completion, increases response rates and improves user experience, especially when sending a campaign to an existing database.
If all required fields are already filled, the form can even be skipped entirely.

Master Forms
To standardise data collection, Qualifio provides Master Forms. They help teams follow a consistent data-collection strategy and prevent configuration mistakes.
Only Admin users can create or edit them. Once attached to a campaign, any modification made to the Master Form automatically updates all campaigns using it.
They can also lock specific settings (for example SSO authentication or form position) so they cannot be changed by other users.

Conditional forms
Long forms can discourage participants. To avoid this, Qualifio offers conditional fields: fields remain hidden by default and appear only when relevant based on previous answers.
The feature works with closed questions and date fields and requires activation in advanced settings section on your account.

Collecting the participant IP address
Qualifio also allows collecting the participant’s IP address through a hidden form field. The field is not visible to users but stores the IPv4 address when the form is submitted.
IP data may also appear in participation reports, and it can be mapped to integrations. Because IP addresses are considered personal data, their collection must respect GDPR requirements.
